Ontario is enhancing its support for long-term care by implementing nurse-led outreach teams, a collaborative effort led by the Nurse Practitioners Association of Ontario (NPAO). This initiative is aimed at delivering personalized care to seniors residing in long-term care facilities.
Under the guidance of Nurse Practitioners, these teams provide tailored healthcare interventions to ensure residents receive timely treatments when needed. With the expertise to diagnose and prescribe, Nurse Practitioners play a crucial role in managing residents’ health and reducing their dependence on emergency care services.
